THE NEW PATIENT CHECKLIST

When you make your appointment, you will be asked to bring:

  • Photo ID

  • Insurance card(s)

  • Registration Forms

 NEW PATIENT REGISTRATION FORMS

Community Health Centers of Lane County has made it easier for first time patients to establish care by offering new patient registration forms online. Simply print the documents and fill them out at your own pace!

Pediatric New Patient Registration Forms (ages 0-18)

1. Call 541-682-3550 to schedule a new patient pediatric appointment.
2. Print pediatric registration packet and complete at your own pace.
3. Bring the completed packet to your child's scheduled visit.

1. Print adult registration packet and complete at your own pace.
2. Drop off the completed packet at any of our clinic locations.
3. Staff will schedule your new patient appointment when they receive your forms.
